Donegal Older Persons Services in Ireland are looking for a
Locum Consultant in Consultant in Geriatric Medicine ASAP - 12 months
Excellent Salary €238,221- €286,151 per annum
**Active IMC Specialist Registration must be in place to apply**
Clinical Duties:
- Provide consultant-led specialist Geriatric Medicine services across the Community Hospital, community services and Letterkenny University Hospital.
- Lead Comprehensive Geriatric Assessment (CGA) and multidisciplinary management planning for older persons with complex medical, functional, cognitive and social needs.
- Support the delivery of specialist frailty, rehabilitation and restorative care services.
- Work with multidisciplinary teams to optimise patient outcomes and maximise independence.
- Provide specialist assessment, diagnosis, treatment and ongoing management of older persons across the continuum of care.
- Support admission avoidance initiatives and facilitate early discharge from acute hospital settings where clinically appropriate.
- Collaborate with General Practitioners, Medical Officers, Psychiatry of Old Age, ICPOP and other specialist services to ensure coordinated and integrated care.
- Attend Letterkenny University Hospital as required to provide specialist geriatric input and support integrated care pathways between acute and community services.
- Promote ageing in place and support older persons to remain in their own homes wherever safe and appropriate.
