The Role:
This role is responsible to the Director of Rehabilitation and Aged Care Services for the provision
of consultant Geriatric and/or Rehabilitation Services to Tamworth Rural Referral Hospital (TRRH) and Peel and Mehi Clusters within HNELHD. You will take responsibility for the following:
Clinical
- Provide an outpatient/community/clinic based Geriatric and/or Rehabilitation consultation service to communities within Tamworth, Peel and Mehi Clusters
- The provision of an effective system of care which is routinely audited and assessed through peer review
- Active participation in organisation wide imperatives such as Between the Flags, Time Out, Clinical Handover
- Effective management of patient access to definitive care, as detailed in DOH guidelines on waiting list management
- Provide an inpatient Geriatric and/or Rehabilitation consultation service to acute and sub acute health facilities within Tamworth Rural Referral Hospital and Peel and Mehi clusters
- Provide support and advice for General Practitioners, Visiting Medical Officers and Residential Aged Care Facilities
- Provide support and advice for Aged Care Assessment Teams (ACAT), Aged Care Services in Emergency Teams (ASET) and Acute to Aged Related Care Services (AARCS) workers
- Provide phone advice when in Armidale based Geriatrician is on leave and the relieve Tamworth based Rehabilitation Physician
- To participate in after hours on-call roster for Tamworth Rehabilitation Unit
- Participate in departmental clinical meetings including audit, grand rounds or physician meetings
- Participate in appropriate clinical activities as determined by the Director
Managerial
- Involvement in the development of policies and procedures required to ensure safe, efficient and effective delivery of patient care
- Ensure delivery of prompt and appropriate service to other Tamworth Hospital departments
- Participation in quality management/accreditation within the Rehabilitation and Aged Care Services
- Establishment, implementation and review of management systems which will support the efficient function of Geriatric/Rehabilitation Services
- Participate in relevant Committees or sub-committees of HNELHD and/or Tamworth Hospital for Mehi or Peel Clusters and where appropriate NSW Health
During the course of providing Geriatric and/or Rehabilitation Medical Services the
Consultant will interact with:
- A multidisciplinary team environment including medical, allied health and nursing staff of the various hospitals to maintain care within various departments and handover to receiving units
- Specialist and General practitioners to receive and provide advice on the management of patients presenting to the Geriatric/Rehabilitation Service
- Medical Administration in relation to roster and leave arrangements
- Patients, carers, families and colleagues and will need to demonstrate a sensitive approach and clear communication with patients, their family and carers, other healthcare professionals and the public
- Commitment to working as an integral and effective member of an Aged Care Team in an open and collegial manner
