An exciting opportunity exists for a Geriatrician to join the newly established Aged Care Outreach Service (ACOS) within the Western NSW Health District.
This role is ideal for Consultants looking to develop a new model of care in a rural context, providing outreach support for older people at risk of hospitalisation across multiple community and residential settings.
- Innovative outreach role: Deliver specialist geriatric care via a hybrid model of face-to-face and telehealth consultations across the region.
- Multidisciplinary team: Collaborate with nurse practitioners, registered nurses, pharmacists, and administrative staff, as well as hospital and community teams.
- Professional growth: Opportunities for learning, development, and collegial support through a partnership with Sydney Local Health District Geriatric Medicine Service.
- Salary package $354,479 - $516,949 pa (commensurate with experience)
The Role
The ACOS provides specialist support to older adults across the region, covering about one-third of the state. The service is designed to reduce hospitalisations for vulnerable patients by providing timely outreach care in residential, community, and hospital settings.
The service operates Monday to Friday, initially 0800–1700, with the potential to extend hours as the service develops. The Geriatrician will be part of a supportive team, benefiting from a partnership with other Health District Geriatric Medicine Services for outpatient services and professional support.
As a Geriatrician you will:
- Provide specialist input for older people in Residential Aged Care Homes, Multipurpose Services, and community settings at risk of hospitalisation.
- Work collaboratively with acute care teams, general practitioners, and allied health staff.
- Deliver outpatient clinics, acute care consultations, and outreach support to develop and improve service delivery.
- Contribute to teaching, research, quality improvement, and service development initiatives.
- Participate in an on-call roster as required.
Requirements
To be eligible, you must:
- Hold Fellowship of the Royal Australasian College of Physicians (FRACP) or
- An equivalent overseas specialist qualification (CCT – UK, CSCST – Ireland, ABIM – USA, RCPSC – Canada, Vocational Registration – NZ) with a recent substantially comparable RACP college outcome
- Be eligible for specialist registration with the Medical Board of Australia (AHPRA)
- Demonstrate strong clinical, teaching, and communication skills
